CONSENT

regarding processing of personal health data

 

I agree that ADB Compensa Vienna Insurance Group (company code: 304080146, address: Ukmergės g. 280, 06115 Vilnius, Lithuania www.compensa.lt, tel.: 19111) will process my health data, as a person insured under voluntary health insurance, for the purposes of investigating and administering insured events or events that may be recognized as insured. I agree that ADB Compensa Vienna Insurance Group may disclose my personal data, including health data, to experts and other persons with special knowledge when this is necessary to determine the fact, consequences and amount of the insured event. I also agree that ADB Compensa Vienna Insurance Group may receive my personal data, including health data, from all healthcare professionals and healthcare institutions for the purpose of investigating and administering insured events or events that may be considered insured.

By giving this consent, I confirm that I am informed that:

- my consent to the processing of personal health data for the above purpose will be valid for the period of validity of ADB Compensa Vienna Insurance Group health insurance coverage and for a period of 1 year after the expiry of the health insurance coverage or until the consent is revoked;

- I have the right to withdraw my consent to the processing of my personal data for the above purpose at any time by contacting the ADB Compensa Vienna Insurance Group customer service by phone at 19111 or by e-mail at zalos@compensa.lt;

- I have the right to request access to my personal data, to request rectification or erasure, or to restrict data processing, and to object to data processing and the right to withdraw my consent to data processing, as well as the right to data portability; if my rights are violated, I may contact the Data Protection Officer by e-mail dpo@compensa.lt, as well as lodge a complaint with the State Data Protection Inspectorate or the court.
