Memo of application for reimbursement

Only the Insured to whom health care services have been provided may apply for the reimbursement of health insurance costs - treatment, counselling, diagnostic tests, rehabilitation, purchase of medicines, optics, dentistry, disease prevention, health services and / or goods (if applicable) .

When submitting the electronic application form, please attach the properly scanned documents provided for in the health insurance rules.

The following documents must be submitted with the application for reimbursement:


We will reimburse the costs of health insurance within 30 calendar days (unless otherwise specified in the contract) from the day when we receive all the necessary documents and information relevant to determining the fact, circumstances and consequences of the insured event and the amount of insurance benefit.
