What documents should be submitted to have expenses reimbursed under the 'Outpatient Treatment and Diagnostics' program?

Medical documents:

  • A copy of the doctor’s consultation summary / discharge summary (hospital discharge report).

Financial documents:

  • The invoice (the services provided must be listed on the invoice);
  • Proof of payment – a cash receipt / a copy of the transfer (internet banking statement) / a cash receipt voucher.

What documents must be submitted to receive reimbursement from the 'All Services' / 'Medical Services' / 'Wellness Services' programs for services provided by a psychologist working under individual activity or business license?

Please submit:

  • The invoice (the services provided must be listed on the invoice);
  • Proof of payment – a copy of the transfer (internet banking statement) / cash receipt voucher / service (goods) purchase and sale receipt;
  • A copy of the individual activity certificate / business license.

Is it necessary to specify the health insurance program from which I expect reimbursement?

In the expense reimbursement request, you may indicate the health insurance program from which you expect reimbursement, but it is not mandatory. We recommend specifying the name of the health insurance program in cases where reimbursement is possible from more than one health insurance program.

What reimbursement amount should I enter, or will it be calculated automatically?

Enter the desired reimbursement amount in accordance with the limit of the health insurance program from which you expect reimbursement and other conditions of your employer’s insurance policy. If you specify a higher amount, we will reimburse the maximum amount allowed under the health insurance policy conditions.

How long does it take to receive the reimbursement?

The processing time for the request depends on the terms of the health insurance contract with your employer. Once a decision regarding the reimbursement of expenses has been made, you will receive a notification via the email you provided when filling out the reimbursement request.

I forgot to attach the document to the request. What should I do?

If you forgot to attach the documents to the request, you can send them via email to sd@compensalife.lt, indicating the request number in the subject line of the email.

How to fill out the application for critical illness?

  • You can find the application form for critical illness here.
  • If, according to the terms of your insurance contract, expenses incurred for the treatment of the illness are reimbursable under Critical Illness Insurance, you can submit your application through the "Compensa Life Lithuania" health insurance mobile app. When filling out the reimbursement application, please indicate in the "Additional Notes" field that you wish to have the expenses reimbursed from the "Critical Illness" insurance.
  • You can send the completed application form for the insurance payout, signed with a certified electronic signature or signed in writing and scanned, to the email sd@compensalife.lt
  • You can also fill out the application for payout/reimbursement at our customer service units, the addresses of which you can find here.

How to fill out the reimbursement application if I do not have online banking and Smart-ID?

  • You can submit the reimbursement application by logging into the E-health self-service and selecting "Submit application without logging in via the online banking system."
  • When sending the completed application form for the insurance payout, signed with a certified electronic signature or signed in writing, to the email sd@compensalife.lt. You can find the application form here.
  • You can also fill out the reimbursement application at our customer service units, the addresses of which you can find here.

We have electronic cards. Where can I find mine?

You can find your electronic card in the "Compensa Life" health insurance mobile app or by logging into the E-health health insurance self-service. In the "Cards" section, select "Download card.

Where should I turn if I lost or damaged my card?

If you have lost your health insurance card, please inform us as soon as possible via email at sd@compensalife.lt, providing your name, surname, the company you work for, and your contact phone number. You can also inform us about the loss or damage of the card by calling +370 5 250 4100 (Monday to Thursday from 8:00 to 17:00, and Friday from 8:00 to 15:45).

Where can I get more information about the terms of my insurance contract?

You can conveniently check and monitor your insurance programs and their remaining coverage amounts in the 'Compensa Life' health insurance mobile app or in the E-health self-service system.

Descriptions of health insurance programs can be found in Appendix No.1 of the Health Insurance Rules.
